During your first visit, we will go over all of the new patient paper work. There is a lot! Please try to have it filled out and back to us 3 - 7 days before your first appointment, along with any recent labs you may have.
Please fill out the New Patient Form along with the Health Appraisal Questionnaire. When completed please email or drop it off to us before your visit.
Insurance: Payment is due at the time of service. We are not a provider for any insurance. Most insurances will not cover nutrition consultations, but we can create an invoice with the ICD-10 codes if you wish to try to be reimbursed.
